What’s the difference between the forms for the different templates below?
All of the forms for one template contains the same fields as its other template equivalent, the only difference is that on the NHS template, the online form is split in to sections and the last section of the form contains a summary list that allows the patient to review what they have entered before submitting the form to you. This is a standard feature on all forms on NHS and .Gov.uk websites.
How to Import
Download the form you want by simply clicking the link below, this will download a zip file to your computer.
Right click on the file that has been download and ‘unzip’ the archive. This will extract a .json file (usually in the same destination as your zip file).
Log in to your website dashboard, click “Forms” on the left hand side and then select “Import/Export”.
Select “Import Forms” and then browse your computer for the .json file.
Once this has been uploaded, you will see a confirmation message and you will see the new form in the list if you go back to the main “Forms” page.
